Information About Fees And Insurance

The cost of an intake is $175. Intakes usually last about an hour. 45-50 minute sessions are $155 and 55-60 minute sessions are $175. We can discuss which is most appropriate. The cost of testing varies based on a person's age and the concerns you have. I can give you an estimate during our initial phone consult or during the intake after I have had a chance to learn more about your concerns.

I am a self-pay or out of network psychologist. Payment is due at the time of service. I accept credit cards, health savings cards, checks, or cash. You may want to check with your insurance plan about out of network coverage if you plan to submit claims (see below). You pay me directly and I will provide you with a superbill (a type of receipt indicating date of service, diagnosis, service codes and cost). You submit this to your insurance company to discuss reimbursement. Superbills are generated by my system at the end of the month and are usually ready by the 2nd or 3rd of the next month.

If you are interested in reimbursement from your insurance company, please contact them directly. You may consider asking:

• Do I have out-of-network benefits for psychological intake (CPT code 90791), therapy (CPT codes 90834 or 90837) or psychological testing (CPT codes 96130, 96131, 96136 & 96137). If so, what percentage do you cover?
• Do I have an out-of-network deductible, and if so, how much of the deductible have I met?
• How do I access the reimbursement form? Where do I send that form?
